Counselor Referral

Parents and or guardians, please download the counseling referral form to refer your student to school-based counseling support. Please return the completed form to the front office or via email to our School Counselor at: diego.arias@cvesd.org. Students may also self-refer (see second pdf) and submit a hard copy to the front office or via e-mail.
